There is no FDA-approved dosing for compounded tirzepatide. As a reference, the schedule approved for Zepbound® starts at 2.5 mg once weekly for 4 weeks, then 5 mg once weekly. The dose may be increased in 2.5 mg steps after at least 4 weeks on the current dose, to a maintenance dose of 5 mg, 10 mg, or 15 mg once weekly, with a maximum of 15 mg. The 2.5 mg dose is a starting dose only. Your clinician sets your dose and pace. Informational only.

Compounded tirzepatide is not FDA-approved. Do not use it if you or a family member have had medullary thyroid carcinoma (MTC) or if you have MEN 2. Serious risks include pancreatitis, gallbladder problems, and severe allergic reactions.
